Volunteer Forms

Below is the documentation that all volunteers are required to submit to Earthwatch at least 90 days before their Earthwatch expedition start date.

  • If completing forms electronically: Save the pdf to your machine before beginning. Use the saved copy to enter your details.
  • Please ensure you complete the required Volunteer Forms in full.
  • If you leave fields unfilled we will ask you to resubmit the document.
  • Please submit your Earthwatch Participation Form as a single document, submitting sections separately will not be accepted.
  • Please note that the Earthwatch Participation Form requires the signature of a doctor to whom you are not related.

At the bottom of this page is a guide to whether you need to complete the Water-Based Projects section in your Earthwatch Participation Form.


Adults (except Scuba Divers)

Self-Certification (only for a small number of projects. Your expedition confirmation email will clearly state if you need this form)

Do I need to complete the Water-Based Projects section?

All Earthwatch Participation Forms include a Water-Based Projects Section. It is not required for all projects but is required for many.

This section is split into two parts:

1) Comfort Level in Water and Boat Experience
2) Snorkel Experience

Some projects require only the Comfort Level in Water and Boat Experience part to be completed. The Snorkel Experience part is only required for projects where snorkelling is involved.

Below is a table that shows the projects that require volunteers to complete some or all of the Water-Based Projects Section in their Earthwatch Participation Form.

If your project is not listed you do not need to complete the Water-Based Projects Section.
